HEALTH BOARD PROVIDES SERVICES IN OUR HOSPITAL POLYCLINIC BUILDING.
For the first applications of our hospital's Health Board transactions (For Work, Gun License, Guardian Appointment, etc. status reporting reports) between 08.30 / 16.30 on weekdays in person; (Disabled, ÇÖZGER Reports) Gun License / Status Reporting Reports) applications are made by making an appointment via 182 MHRS.
THINGS TO BE CONSIDERED IN HEALTH BOARD TRANSACTIONS:
Our hospital's Health Board meets twice a week on Wednesdays and Fridays.
Check the information in the examination document given to you for your board transaction (identity information, address, phone number, photo, etc.) to make sure that the information is correct and that the document belongs to you.
In order for the board procedures to be initiated, the person for whom the report will be prepared must apply to the Health Board with their ID on the appointment day and time. If the person does not have an ID or the person themselves, the procedure will not be initiated.
If the patient is under the age of 18, the patient's mother, father or legal guardian must be with the patient during the health board procedures.

After completing your board examination procedures, you must submit all your relevant documents (analysis, examination, drug report, epicrisis report, etc.) to the health board unit. If you think you will need any documents later, please make a photocopy. At the end of the procedure, the documents will not be removed from the documents submitted and will not be given.
If the examinations requested from you during your examination procedures will take a long time, inform the Health Board Unit.
